The Embrace Hospice House is a licensed facility providing short-term General Inpatient Level of Hospice Care ("GIP") for pain and symptom management, as well as Routine Level of Hospice Care ("Residential") and Respite Care.  Medical care staff will be coordinated by the Facility's Medical Director.  Additional services are provided by a skilled interdisciplinary team of professionals, including:  Physician(s), Nurse Practitioner(s), Registered Nurse(s), Licensed Practical Nurse(s), Social Worker(s), Certified Nursing Assistant(s), Spiritual Care Coordinator(s), and Volunteer(s).  Care is provided twenty-four (24) hours a day, seven (7) days per week.

Embrace Hospice House has several contracts with local hospice providers for facility beds.  In the event that you already have hospice care for your loved one, please inquire with your hospice agency about their access abilities with the Embrace Hospice House.  You can always call us directly and we'll be more than happy to discuss how to get your loved one into our facility!

Short-Term General Inpatient Level of Care ("GIP").   This level of care includes medical, nursing, and psychosocial care for pain control and/or other symptom management.  GIP charges are covered by the Medicare hospice benefit, Medicaid, and/or some private insurance policies.   While on GIP, Room and Board charges (single-occupancy room, meals, television, and local phone calls) are covered by Medicare, Medicaid, and/or some private insurance policies.

Patients and/or representative(s) are invited to participate in care planning meetings at least weekly.  Once pain control and/or other symptoms are managed, the patient's care status must be changed to Residential, and there will then be facility Room and Board charges.  If continued residence at the facility becomes inappropriate according to Medicare guidelines, assistance with relocation will be provided for the patient with patient and/or representative(s) cooperation and involvement.
Routine Level of Care ("Residential").  This level of care includes medical, nursing, and psychosocial hospice care services.  Residential charges except for Room and Board are covered by Medicare, Medicaid, and/or some private insurance policies.

While on Residential care, Room and Board charges (for single and double-occupancy rooms, meals, television, and local phone calls) are not covered by Medicare, Medicaid, and/or some private insurance policies.

A patient's care needs may fluctuate between qualifying for GIP and Residential (per Medicare/Medicaid guidelines) during a period of time at the facility.  If placed on Residential care, Room and Board charges will be $236.00 per day for a single-occupancy (private) room and $184.00 per day for a double-occupancy (semi-private) room.
